Valid government-issued ID and a fully accomplished Application Form.

Membership application is FREE.
Restricted Persons
Guests cannot complete sign-up if they are classified as restricted personnel, including: Politically Exposed Persons (PEP), government officials, military/police personnel, students, PAGCOR license holders, self-excluded individuals, and LaVie employees.
How to activate your LaVie Rewards card?
- After linking on the Casino Plus App, proceed to any LaVie Rewards Counter to fill out the Membership Application Form.
- Fill out the Membership Application Form along with a valid government-issued ID.
- LaVie Rewards membership card will be issued.
Card Matching with Casino Plus
Card Matching is awarded on an honorary basis. Based on tier upon sign-up. Card validity is 1 year.
